If your Student Agreement and your account are current you may request a degree change by completing and submitting this form. Once the form is received, your degree program change options will be emailed to you from the Office of Student Records. 

If your Student Agreement and your Student Account are not current, DO NOT submit this form. Student Reactivation will be required first.
