TO APPLY FOR GRADUATION - 1) STUDENT AGREEMENT MUST BE CURRENT 2) STUDENT ACCOUNT MUST BE CURRENT 3) GRADUATION FEE MUST ACCOMPANY THIS FORM

THIS FORM WILL NOT BE PROCESSED IF: 1) The Graduation Fee is not included. 2) Your Student Agreement is expired or will expire prior to April 15, 2026. 3) Your Student Account is past due or delinquent. 4) You have been enrolled for less than 3 full months. 5) The form is incomplete in any way. An incomplete or invalid form is not considered "received" for purposes of the March 25, 2026 deadline. Submit Early to allow time to resubmit form deemed "invalid" by the deadline.

GRADUATION DEADLINES (Extensions are Not Available - Do Not Inquire)

March 25, 2026 - Graduation Fee/Graduation Form must be received by the NTS office. Forms and/or graduation fee payments received after this date will not be processed. Include complete credit card information when submitting the form or the form will not be processed and will not be considered "received". If you print and mail the form, you must either include complete credit card information or your check. DO NOT SEND ANYTHING REQUIRING A SIGNATURE - IT WILL NOT BE CLAIMED!

April 15, 2026 - All dissertations, ministry projects and assignments required to complete your degree program are due. Anything received after this date will delay your graduation until at least December 2026. THESE MUST BE EMAILED - DO NOT MAIL - THEY WILL NOT BE PROCESSED!

***Graduation Fees are non-transferable and non-refundable. Do not complete this form if you are not certain you will be able to complete the degree program requirements and pay your account balance in full by the specified deadline. If you register for graduation and do not graduate, for any reason, the graduation fee will be forfeited. If you register to attend graduation and do not attend, for any reason, your degree will be shipped. Partial refunds are not available.***
